On May 10, an informative and impactful information session took place, providing updates on the remarkable work being carried out by VCSQI in partnership with the Virginia Hospital and Health Association (VHHA) in Virginia. The session aimed to shed light on our collaborative efforts in developing comprehensive guidelines and resources aimed at reducing the readmission rate for cardiac patients across the state.
The partnership between VCSQI and VHHA represents a powerful alliance focused on addressing a critical healthcare challenge. By combining our collective expertise, research capabilities, and passion for improving cardiac care, we have embarked on an ambitious journey to develop guidelines and resources tailored to the specific needs of cardiac patients. Our goal is to ensure a seamless continuum of care, from hospital discharge to post-discharge follow-up, with the ultimate aim of reducing readmissions and enhancing patient well-being.
