Reducing preventable hospital readmissions is a national priority for payers, providers, and policymakers seeking to improve health care and lower costs. In 2022, the Virginia Cardiac Services Quality Initiative (VCSQI) in partnership with the Virginia Hospitals and Healthcare Association (VHHA) launched a readmission workgroup, led by Dr. Robert Lancey of Sentara Rockingham Memorial Hospital to identify and discuss efforts taking place in their member organizations to reduce preventable hospital readmissions.

This webinar will feature presentations from VCSQI and VHHA representatives as well as presentations from hospitals across Virginia who will describe operational strategies that have contributed to improved readmission rates. It will also highlight new improvement opportunities.

The intended audience for this webinar includes providers, quality and patient safety staff, care managers, patient navigators, health coaches, clinical care team, transition care staff, and other readmission reduction hospital team members.

This webinar is free and open to all providers and staff at VHHA member hospitals and health systems and VCSQI members.
