SDOH Screening Initiative: Your Input Needed

We are launching a statewide initiative to strengthen and standardize Social Determinants of Health (SDOH) screening and operational use across participating sites. Current challenges include low screening completion rates, inconsistent clinic-to-inpatient data transfer, limited use of Z-codes, and missed opportunities to use SDOH data to meaningfully guide patient care.


This project is being led by MD/MPH Candidate Manny Lee as part of his practicum. The goal is to develop a practical, statewide SDOH Best Practices Toolkit outlining recommended workflows for outpatient and inpatient screening, EMR data flow, Z-code utilization, and strategies for closing the loop with case management and community resources.


To inform this work, we are asking sites to complete a brief survey to share current workflows, barriers, and opportunities for improvement.



If you are not the appropriate person to complete the survey, please forward it to a team member who oversees or manages SDOH screening at your program.


Your input will directly shape statewide recommendations and help ensure SDOH data is operationalized to improve care delivery and advance health equity.

Strengthening ECMO Care Across Virginia — Join the VCSQI ECMO Workgroup


Extracorporeal Membrane Oxygenation (ECMO) continues to grow as a lifesaving support modality for patients with severe heart and lung failure. To advance care quality and consistency in our region, the VCSQI ECMO Workgroup is hosting a special collaborative session on Thursday, March 26, 2026, from 3:30 PM–4:30 PM at The Oak Boardroom – The Kimpton in Charlottesville, VA.


This event brings together ECMO leaders, clinicians, and multidisciplinary team members from across the state to share best practices, align care processes, and build a stronger statewide network of support.


The session will feature a detailed look at VCU’s ECMO Program Structure, presented by Brandi Bummer (VCU), offering insights into operational frameworks that support high-quality extracorporeal care.
