Advancing Collaboration and Standardized Care Across Virginia
The Virginia Cardiac Services Quality Initiative (VCSQI) is proud to support our partners at the Virginia Heart Attack Coalition (VHAC) in announcing the release of the new Pulmonary Embolism Response (PERT) Protocol — a statewide, evidence-based framework designed to improve the recognition and management of acute pulmonary embolism.
Developed by the VHAC PE Response Workgroup, a multicenter collaborative led by Dr. Michael Kelley of Carilion Clinic, this protocol reflects a unified effort among hospitals and clinical teams across the Commonwealth to bring consistency, speed, and precision to PE response.

Rapid Activation Criteria – Clearly defined, evidence-based triggers for initiating the PERT response
Standardized Care Pathway – Guidance for PE evaluation, anticoagulation, and escalation of care based on risk level
Defined Roles and Workflow – Seamless coordination among ED, hospitalist, critical care, and interventional teams
Statewide Consistency – Aligns institutions under a shared framework for quality care and reporting
Introducing the VHAC PE Data Collection Form
Turning Coordination into Measurable Impact
To complement the clinical protocol, the VHAC PE Response Workgroup has also developed the VHAC PE Data Collection Form, a secure, web-based submission tool that allows member institutions to record PERT activations and outcomes in real time.
This tool enables statewide data aggregation, helping identify trends, inform best practices, and guide future quality-improvement and research efforts.
